Medical Form
Each September, Pack 318 requests that a new BSA Medical Form (Parts A & B) be completed by the scout's parents/guardians.
Providing your medical information on this medical form will help ensure you meet the minimum standards for participation in various activities. And that any special medical conditions are known (e.g., alergic reactions to nuts). Note that unit leaders must always protect the privacy of unit participants by protecting their medical information.
This medical form is provided as a fillable PDF that can be downloaded below. Members are encouraged to fill it out on their computer, then print the record (rather than printing the record and filling it out by hand). Doing this will improve the readability and accuracy of each member's medical information.
Parts A and B are to be completed at least annually by participants in all Scouting events.
Part C is the physical exam that is required for participants in any event that exceeds 72 consecutive hours, for all high-adventure base participants, or when the nature of the activity is strenuous and demanding. These will only be requested for special event actvities such as Summer Resident Camp at Ravenknob, or Webelos II 3 day trip to Patriots Point. Our normal weekend family camping events will not require Part C.
